Rationale: Autologous stem cell transplantation is a key therapy for hematologic malignancies such as multiple myeloma, relapsed NHL, and Hodgkin lymphoma. The procedure is often associated with prolonged hospitalization, fatigue syndrome, deconditioning with loss of muscle mass, and an increased risk of complications. Most prehabilitation studies focus on exercise alone, while multidisciplinary approaches remain poorly investigated. Nutritional assessment and individualized dietetic therapy may help address malnutrition and optimize treatment readiness. There is an urgent need to determine whether combined prehabilitation can improve fitness, enhance recovery, and enable transplantation in patients previously considered unfit.
Methods: A three week multidisciplinary prehabilitation program at the Tisserand Health Centre delivered 1.800 minutes of tailored interventions including resistance training, respiratory therapy, massage, gymnastics, electrotherapy, psychological and dietetic counselling, and education. Each component was adapted to individual needs, and adherence was monitored. Endpoints assessed pre and post intervention included handgrip strength, 6 minute walk test, respiratory muscle performance, health related quality of life, and body composition.
Results: Among six pilot participants, the median age was 62.8 years, five had multiple myeloma, and one had DLBCL. The 6-minute walk distance increased by a mean of 66 m, EQ-5D-5L VAS improved by 18.7 points, and inspiratory muscle strength increased by 3 mbar on average. Adherence to the program was high, and two patients previously deemed unfit became eligible for autologous stem cell transplantation following the intervention.
Conclusion: These preliminary data support feasibility and potential efficacy of the program and justify a larger controlled study to confirm effects.
