Rationale: The clinical significance of myosteatosis in colon cancer remains unclear. This study evaluated the impact of malnutrition, sarcopenia, sarcopenic obesity, myosteatosis, and systemic inflammation on postoperative outcomes, and assessed the prognostic value of the Cancer Cachexia Index (CXI).
Methods: We prospectively analyzed patients undergoing colorectal surgery with curative intent between January 2018 and January 2020. Preoperative body composition parameters, including skeletal muscle index (SMI), visceral fat area (VFA), and mean muscle radiodensity (HU), were assessed by CT at the L3 level. Clinical and biochemical data were collected. CXI was calculated as SMI × serum albumin / neutrophil-to-lymphocyte ratio. Outcomes included postoperative complications (Clavien–Dindo), 90-day mortality, length of stay, discharge destination, reoperation, and readmission.
Results: A total of 100 patients (76% male) with a mean age of 60.1 ± 8.6 years were included. Sarcopenia was present in 76% and myosteatosis in 55.3%. Myosteatosis was an independent risk factor for severe postoperative complications (Clavien–Dindo > III) and prolonged hospital stay. Chronic inflammation was associated with increased risk of myosteatosis. Low CXI was associated with worse postoperative outcomes.
Conclusion: Myosteatosis independently predicts adverse postoperative outcomes in colon cancer surgery. Chronic inflammation is linked to myosteatosis, and CXI may be a useful prognostic tool.
