Rationale: Inadequate nutrition can generate metabolic stress and poor clinical outcomes. The aim of the study to evaluate a structured Nutrition Care Process (NCP) on trauma patients. The objectives are to record assessment scores to establish a NCP framework and analyse clinical outcomes and formulate evidence-based nutrition protocols.
Methods: The prospective interventional study involves 248 adult patients(≥18y) with head and orthopaedic injuries. Participants were divided into two ICU(n=124) with individualized nutritional plans and a non-ICU(n=124) group with standard hospital diets. Comprehensive nutritional and clinical assessments were performed using validated tools, including: SGA, NRS, APACHE IV, SOFA, NUTRIC, GCS. Interventions were dynamically tailored based on baseline and follow-up clinical and anthropometric phenotypes.
Results: Baseline assessments revealed that ICU patients exhibited significantly higher disease severity and nutritional risk, characterized by elevated SGA, APACHE IV, SOFA, and NUTRIC scores, alongside lower GCS scores compared to the non-ICU cohort (p<0.05). Notably, 15.32% of ICU patients were classified as high risk via NUTRIC scores, compared to only 0.81% in the non-ICU group.

Conclusion: Trauma patients in the ICU showed pronounced metabolic derangement and inflammatory profiles that require specialized nutritional intervention. While injury severity remains a dominant predictor of mortality, the implementation of a structured NCP is essential for recovery from physiological deficits and improving clinical management.
