Rationale: Disease-related malnutrition (DRM), particularly when associated with inflammation and morphofunctional impairment, may negatively impact hospital survival. Identifying high-risk clinical phenotypes could improve prognostic stratification and guide targeted interventions.
Methods: A total of 695 hospitalized patients from the multicenter DRECO study were analyzed. A myoinflammatory phenotype was defined by the coexistence of inflammatory markers (CRP >5 mg/dL or bioimpedance parameters [BIA Akern® Nutrilab]: phase angle <1.65 and Na/K ratio >1.2) and muscle dysfunction (dynapenia: women <16 kg; men <27 kg). Kaplan–Meier survival curves at 3 and 6 months were estimated. Univariable and multivariable Cox regression models adjusted for age and sex were performed.
Results: The myoinflammatory phenotype with functional impairment was associated with reduced survival from early follow-up: 3 months: 85.6% vs 96.0%; 6 months: 73.7% vs 92.5%. In multivariable analysis, this phenotype was an independent predictor of mortality (HR: 3.26; 95% CI: 2.11–5.05; p < 0.001), after adjustment for age (HR: 1.02; p = 0.011). Sex was not significant.

Conclusion: The myoinflammatory phenotype with functional impairment is independently associated with lower 6-month survival in hospitalized patients with DRM or nutritional risk. Early identification may help prioritize targeted nutritional and functional interventions to improve prognosis.
