Rationale: The use of oral nutritional supplements (ONS) has been shown to improve nutritional status in patients with cancer, being adherence a key factor for its effectiveness. This study aims to evaluate efficacy and adherence of a nutritional support program including a high-protein (32g/200mL) and high-calorie (≥2.1 kcal/mL) ONS.  

Methods: ONAVIDA is an observational, prospective, non-interventional cohort study conducted in 18 Spanish hospitals. A total of 230 patients were recruited. Patients were divided into 147 naïve and 83 non-naïve. Nutritional status was evaluated using GLIM criteria and morphofunctional assessment. Quality of Life (QoL) was assessed using NutriQoL®. 

Results: Mean (SD) age was 64.38 (11.1) and 59,8% of patients were male. Head and neck cancer was the most diagnosed, 84 (36,7%; N=229).Around 82 (37,1%; N=221) patients had stage IV  cancer. ECOG 0 was reached by 71 (30,9%; N=230) at baseline. According to GLIM criteria, statistically significant results were reported: after 3 months of the highly concentrated ONS, 23,8% returned to their normal nutritional status. Additionally, Timed Up and Go test showed a statistically significant improvement in their functional capacity by reducing the time 1.3 seconds. Muscle strength (dynamometry) increased significantly (+1.77kg) at 3 months. Based on Wanden-Berghe Scale, 148 (88,1%; N=180) of patients reported adherence to the ONS. Non-naïve reported a change of over 4 points on the NutriQoL® indicating a clinically significant enhancement in QoL. Most patients did not present any gastrointestinal symptoms.

Conclusion: A nutritional support program incorporating a high-protein, high-calorie concentrated ONS significantly enhances the nutritional status of patients  with cancer and malnutrition and achieves a high level of adherence.
