Rationale: Systemic venous congestion is a key driver of outcomes in cardiorenal syndrome. While the Venous Excess Ultrasound Score (VExUS) allows objective assessment, it is operator-dependent. Preliminary data suggest a possible association between bioelectrical impedance vector analysis (BIVA) and ultrasound congestion markers, but evidence remains scarce. We aimed to explore whether BIVA could contribute to congestion phenotyping by assessing its association with VExUS.
Methods: Observational study including 28 stable patients with cardiorenal syndrome. BIVA parameters included resistance (R), reactance (Xc), phase angle (PhA), standardized phase angle (SPA), body cell mass (BCM), body cell mass indexed to height (BCM/H), total body water (TBW), extracellular water (ECW), hydration index (ECW/TBW), fat-free mass (FFM) and fat mass (FM). VExUS score was assessed. Associations were analyzed using Spearman correlation. Patients were stratified into low (≤1) and high (≥2) congestion groups and compared using Mann–Whitney test.
Results: VExUS showed an inverse correlation with phase angle (r = −0.23) and reactance (r = −0.26), and a positive correlation with hydration index (r = 0.26) and extracellular water (r = 0.21). Patients with higher VExUS presented lower phase angle and increased hydration parameters. Although not statistically significant, all trends were directionally consistent with higher congestion states.

Conclusion: BIVA parameters showed consistent directional trends with ultrasound-defined venous congestion. Although not statistically significant, these findings suggest a potential association and highlight BIVA as a promising bedside approach for congestion assessment, warranting confirmation in larger studies.
