IMPACT OF GLP-1 RECEPTOR AGONIST THERAPY ON IVF OUTCOMES IN WOMEN WITH POLYCYSTIC OVARY SYNDROME: A PROSPECTIVE RANDOMİZED COMPARATIVE STUDY

Rationale: GLP-1 receptor agonists (GLP-1 RAs) have emerged as promising metabolic modulators; however, their impact on assisted reproductive technology (ART) outcomes remains unclear in Polycystic ovary syndrome (PCOS). This study aimed to evaluate the effects of GLP-1 RA therapy on ovarian response, embryo development, and clinical pregnancy rates in women with PCOS undergoing in vitro fertilization (IVF). 

Methods: In this study, 100 women with PCOS undergoing IVF were included and randomly allocated into two groups: metformin-treated group (n=50) and GLP-1 RA–treated group (n=50). Main outcomes included mature (MII) oocyte count, fertilization (2PN) rate, and day-5 embryo development and clinical pregnancy rate.

Results: Baseline demographic and hormonal parameters were comparable between groups (p>0.05), except for a higher baseline antral follicle count in the GLP-1 RA group (25 ± 6 vs. 20 ± 6; p<0.001). The GLP-1 RA group demonstrated significantly improved IVF outcomes, including higher numbers of MII oocytes (19 ± 5 vs. 15 ± 5; p<0.001), 2PN embryos (17 ± 5 vs. 12 ± 5; p<0.001), and day-5 embryos (7 ± 3 vs. 5 ± 3; p=0.0009). Clinical pregnancy rates were significantly higher in the GLP-1 RA group compared with the metformin group (78% vs. 56%; p=0.033), corresponding to a risk ratio (RR) of 1.39 (95% CI 1.05–1.85). GLP-1 RA therapy was associated with increased odds of clinical pregnancy (OR 2.79; 95% CI 1.17–6.66). 

Conclusion: In this first prospective randomized clinical trial, GLP-1 receptor agonist therapy significantly improves ovarian response, embryo development, and clinical pregnancy rates in women with PCOS undergoing IVF compared to metformin.
