CLINICAL PHENOTYPES OF PATIENTS RECEIVING 3-IN-1 PARENTERAL NUTRITION IN A NATIONAL HOSPITAL IN PERU: A CLUSTER ANALYSIS

Rationale: We aimed to identify distinct clinical profiles among patients receiving 3-in-1 parenteral nutrition (PN).
Methods: A retrospective multivariate observational study was conducted on 116 hospitalized patients who received 3-in-1 PN during 2024 at a national hospital in Peru. Variables analyzed included sex, age, hospital ward, nutritional diagnosis, PN duration, renal failure, and in-hospital mortality. A hierarchical cluster analysis was performed using Ward’s method and squared Euclidean distance. Four optimal clusters were identified from the dendrogram.
Results: The mean age was 56.3 years (SD ±17.9), with 58.6% males. Half were admitted to the ICU. Indications were intestinal failure (86.2%) and gastrointestinal tract injury (13.8%). Mean PN duration was 11 days (SD ±10.65). Renal failure occurred in 23.3% and mortality was 29.3%. Cluster 1 included critically ill patients with high mortality (74.2%) and renal failure (77.4%).Cluster 2 comprised only stable male patients with no organ failure or deaths. Cluster 3 included females with intermediate risk and 27.8% mortality but no organ dysfunction. Cluster 4 involved patients with intestinal injury, low mortality (6.3%), and variable outcomes. These profiles reflected different clinical and nutritional risk levels.
Conclusion: Hierarchical cluster analysis enabled the identification of four distinct clinical phenotypes among patients receiving parenteral nutrition.
