PHASE ANGLE IS MORE STRONGLY ASSOCIATED WITH MUSCLE STRENGTH THAN PRE-SARCOPENIC OBESITY IN YOUNG ADULTS

Rationale: Phase angle (PhA) is a marker of cellular integrity and muscle quality, but its association with muscle function and adiposity in young adults is not fully understood. This study aimed to investigate the association between PhA and muscle strength, body composition, and pre-sarcopenic obesity (Pre-SO) in young adults.
Methods: This cross-sectional study was conducted on 166 healthy young adults (89.2% of female, mean age: 22.67 ± 3.38 years) in İstanbul. Body composition was assessed using bioelectrical impedance analysis, and hand grip strength was measured as an indicator of muscle function. Pre-SO was defined using skeletal muscle mass-to-weight ratio and obesity criteria. Data were analyzed using SPSS 24.0.
Results: The prevalence of pre-sarcopenia was 10.2%, of whom 94.1% were classified as Pre--SO. PhA showed significant positive correlations with hand grip strength (r: 0.352, p< 0.001), fat-free mass (r: 0.262, p: 0.001), waist circumference (r: 0.230, p: 0.003), and Body mass index (BMI) (r: 0.180, p: 0.020). BIVA-derived parameters (R/H and Xc/H) showed strong inverse correlations with PhA, body composition, and muscle strength (all p< 0.001), supporting the impedance-based nature of PhA. In multivariate analysis, hand grip strength (β: 0.309, p: 0.001) and BMI (β: 0.189, p: 0.026) were independently associated with PhA, while age, gender, physical activity level, and Pre-SO were not significant predictors. The model explained 19.5% of the variance in PhA.
Conclusion: PhA was more strongly associated with muscle function than with adiposity or pre-SO, suggesting that it reflects functional rather than structural aspects of muscle health in young adults.
