TIME AND MOTION COMPARISON OF THREE DIFFERENT NUTRITION COMPOUNDING SYSTEMS

Rationale: Automated compounding systems for parenteral nutrition differ widely in automation, speed, process safety, and setup. The purpose of this study is the objective quantification of performance and process differences between three commercially available compounders
Methods: Under standardized laboratory conditions, pre-defined within a protocol and notarial deposited, Impromediform Medimix Multi® (12 channels), Baxter ExactaMix® 2400 (EM2400) and B. Braun APEX® have been compared in a production site in Spain, where all machines are in regular everyday use under GMP requirements. To guarantee comparability and real-life setting, a pre-ordered standardized set of compounded nutritional bags for hospital use was produced on three consecutive days, each day on a different system.
Results: For comparison of production gross outcomes (incl. interruptions etc.), the maximum time was set to 8:20h, reflecting 150l limitation of the EM2400 transfer set. In this time, Medimix produced 50 bags (97 l), EM2400 72 bags (143 l) and APEX 82 bags (162 l). This results in favour of produced bags: EM2400 vs. Medimix +44%, APEX vs. Medimix +64%, and APEX vs. EM2400 +14%. Net production time is measured with standardized quality reports. In the shift, Medimix produced a total of 63 bags (124 l) in 9:15h net production time, while EM2400 needed for the same set 6:17h (178min faster and -32% vs. Medimix), while APEX even faster produced the same set in 4:49h, 266min faster, -48% vs. Medimix and 88min faster, -24% vs. EM2400. On individual bag type comparison (by regimen), for 9 of 11 included produced types, APEX has been statistically significant faster versus the other two systems (see graph 1).

Conclusion: The use of B. Braun APEX® system was associated with increased throughput of produced nutrition regimens in volume and number of bags, resulting in improved efficiency and resource utilization to release more time for patient care.
