Rationale: Preconception dietary habits in women undergoing assisted reproductive treatment (ART) may influence ART outcomes. This study investigated the association between adherence to two a-priori defined healthy dietary scores with IVF outcomes in a sample of women undergoing their first IVF treatment.
Methods: In a sample of 244 non-obese women (26-55 years of age; BMI: <30 kg/m2), dietary habits were assessed through a food frequency questionnaire from which two dietary scores were calculated: the Prospective Urban Rural Epidemiology (PURE) diet score (range: 0-6) and the Healthy Eating Index (HEI-2020) (range: 0-100), with higher scores indicating greater adherence to a healthy diet. Multiple logistic regression analysis was used to evaluate associations between tertiles of the two scores and IVF intermediate outcomes (embryo quality measures and fertilization rate) and clinical endpoints (implantation, clinical pregnancy, live birth), after adjusting for potential confounders including age, physical activity, smoking, stress and supplements use.
Results: In total, 229 women underwent embryo transfer, 138 had successful implantation, 104 achieved a clinical pregnancy and 99 gave live birth. No association of PURE and HEI-2020 scores with IVF intermediate outcomes was observed. Compared with women in the lowest tertile of the PURE score, those in the highest teritle of the score had higher rates of live birth (48.5% vs. 26.5%, P=0.04). In multivariate analyses, higher adherence to the PURE and HEI-2020 scores was associated with an increased probability of live birth [odds ratio (95% confidence interval): 2.39 (1.07-5.35) and 2.07 (1.00-4.27), respectively].
Conclusion: Higher adherence to the PURE and HEI-2020 diet scores was associated with an increased chance of live birth in women undergoing ART.
