Rationale: Implementation of a validated nutrition questionnaire within the digital patient portal. To enable early identification of malnutrition, provide timely evidence-based nutritional information, and preventing patients from adopting unbalanced or unnecessarily restrictive diets.
Methods: A literature review was conducted to identify PROM-based tools suitable for evaluating nutritional status. The PG-SGA Short Form was selected as the foundation for the questionnaire. The instrument covers four domains: (1) weight change; (2) dietary intake, including portion size and food consistency; (3) symptoms affecting intake, such as nausea, taste alterations, and dysphagia; and (4) physical activity and functional status, including limitations in daily tasks or bedridden status. The questionnaire was embedded into the Ghent University Hospital patient portal, with automated links to educational resources triggered by patient-reported symptoms.
Results: All patients with a new cancer diagnosis receive the nutritional questionnaire through the hospital’s patient portal. The validated PG-SGA Short Form is used to assess nutritional risk. Patients scoring ≥ 4 are automatically scheduled for a consultation with a specialist dietitian in cancer care. This process allows specialist cancer care dietitians to focus on patients with the most urgent or complex nutritional needs.
Conclusion: The integration and routine monitoring of responses to a validated PROM-based nutritional questionnaire enable timely detection and management of malnutrition in patients with cancer. This automated approach optimizes proactively resource allocation and targeted dietetic care where needed due timing and complexity.
