COST-EFFECTIVENESS ANALYSIS OF 100% WHEY PEPTIDE COMPARED TO STANDARD POLYMERIC FORMULAS IN US PEDIATRIC HOME ENTERAL NUTRITION

Rationale: Medically complex pediatric patients requiring home enteral nutrition (HEN) are often challenged with intolerance. Emerging data has shown peptide-based formulas, especially those containing 100% whey protein and medium chain triglycerides, may enhance tolerability and decrease healthcare resource utilization. The aim of this study is to assess cost-effectiveness of 100% whey peptide-based formulas (100%-WP, Peptamen Junior®) vs. standard polymeric formulas (PF) in US pediatric HEN patients post hospital discharge.
Methods: A 12-month Markov model with daily cycles was developed with US healthcare system perspective, simulating costs and life days for the health states including stable health, oral autonomy, healthcare visits, and inpatient stays. Model inputs were derived from US-specific data sources.
Results: Compared with PF, 100%-WP enteral nutrition was associated with greater time spent in a stable, complication-free health state. Patients receiving 100%-WP spent an average of 197 days per year in this state, compared with 172 days for PF, an increase of 25 days. A higher proportion of patients in the 100%-WP group transitioned from home enteral nutrition to oral autonomy within the model time horizon. Healthcare resource utilization was lower with 100%-WP, including fewer annual healthcare visits (49 vs. 83) and fewer inpatient days (38 vs. 58) compared to PF. Although product acquisition costs were higher, overall annual healthcare costs were lower with 100%-WP, resulting in mean savings of $75,881 per patient (p<0.0001). Probabilistic sensitivity analysis supported the robustness of these findings.
Conclusion: 100%-WP formula use is more cost-effective than standard PF in US pediatric HEN patients. It advances health outcomes, reduces inpatient stays and outpatient visits, generating significant healthcare cost reductions.
