EFFECTS OF DIFFERENT DIETARY PATTERNS ON TYPE 2 DIABETES MELLITUS PATIENTS WITH OVERWEIGHT AND OBESITY:A NETWORK META-ANALYSIS

Rationale: To evaluate the effects of five dietary patterns on patients with T2DM complicated with overweight/obesity using a network meta-analysis, and provide more comprehensive and reliable evidence for dietary and nutritional interventions for them.
Methods: Databases including PubMed, Web of Science, Cochrane Library, CNKI and Wanfang were systematically searched for randomized controlled trials from January 1, 2010 to July 1, 2025, in Chinese and English. Two researchers independently extracted data. Study quality was assessed by Review Manager 5.3 based on Cochrane risk-of-bias tool, and data analysis was performed using Stata 18.0.
Results: A total of 21 studies involving 1,702 patients were included. For fasting blood glucose, intermittent fasting was optimal (SUCRA=91.3%), followed by high-protein diet (65.1%) and ketogenic diet (61.4%). For glycated hemoglobin, ketogenic diet ranked first (SUCRA=89.5%), followed by Mediterranean diet (64.5%) and intermittent fasting (61.1%). For low-density lipoprotein cholesterol, Mediterranean diet was the best (SUCRA=74.1%), followed by intermittent fasting (70.7%) and ketogenic diet (54.1%). Ketogenic diet showed the best improvement in body weight (SUCRA=99.2%) and waist circumference (SUCRA=91.1%).

Conclusion: Intermittent fasting and ketogenic diet effectively improve blood glucose and obesity indicators. Mediterranean diet presents better lipid-lowering effect and is suitable for long-term application. Individualized dietary plans should be formulated according to metabolic status, focusing on sustainability and safety.
