Rationale: Accurate dietary assessment tools are essential for understanding nutritional intake, especially among vulnerable populations. In urban slum settings, particularly among low-literacy women, traditional food frequency questionnaires (FFQs) can be difficult to administer and interpret. Therefore, a simplified FFQ tailored to this population is critical for effective nutritional surveillance and intervention planning.

Methods: A cross-sectional validation study was conducted among 625 low-literacy women aged 15–49 years residing in urban slums near Lucknow, Uttar Pradesh. The simplified FFQ was developed based on commonly consumed local foods and pilot-tested for comprehension. Dietary intake data were collected using both the simplified FFQ and a 24-hour dietary recall (as the reference method) over two non-consecutive days. Statistical analyses included Pearson correlation, Bland–Altman plots, and cross-classification analysis to determine agreement between the two tools.

Results: The simplified FFQ demonstrated moderate to strong correlations with the 24-hour recall for most food groups (r = 0.52–0.78). Bland–Altman analysis indicated acceptable limits of agreement, and 84% of participants were classified into the same or adjacent quartiles of nutrient intake. Misclassification into extreme quartiles was less than 5%, suggesting good validity

Conclusion: The simplified FFQ is a valid, culturally appropriate, and practical tool for assessing dietary intake among low-literacy women in urban slums of Lucknow. Its use can enhance community-based nutrition assessments and support targeted interventions in similar low-resource settings.
