INTRADIALYTIC PARENTERAL NUTRITION (IDPN) A PRACTICAL EXPERIENCE AND LITERATURE REVIEW

Rationale: To report the practice of intradialytic parenteral nutrition in a malnourished patient followed up in the nephrology department of a public hospital in Brazil with the aim of improving their nutritional status and quality of life.
Methods: Review of medical records, comparison of anthropometric and dynamometric data before and during treatment. Treatment was initiated on 06/31/2024 and maintained regularly 3x per week during the hemodialysis process via arteriovenous fistula; The diet was compounded by the hospital pharmacy. Trace elements and vitamins were also infused. The treatment was completed on 11/25/2024. A review of Guidelines and conceptual articles was performed for scientific and practical basis.
Results: At the beginning of treatment, the patient was hypoalbulinemic (2.8 mg/dL) and malnourished, with a BMI of 17.8 kg/m² (initial weight of 56.5 kg). Treatment began on June 31, 2024. At the end of treatment, no changes in weight (54 kg) or BMI (17.4 kg/m²) were observed. Other parameters such as dynamometry, quality of life and albumin improved positively. Metabolic changes such as hypertriglyceridemia or liver changes were not observed. There was no difficulty in glycemic control. The patient was maintained on a bland oral diet with complementary food supplementation.
Conclusion: Although it did not influence the patient's body composition, intradialytic parenteral nutrition still proves to be a major challenge in public health services given the peculiarities of nephropathy patients. More cases and clinical studies are needed to improve the experience of services regarding this important treatment for the patient.
