IMPACT OF DONOR NUTRITIONAL STATUS ON HUMAN MILK COMPOSITION: PILOT STUDY

Rationale: Studies indicate that maternal nutritional status appears to have an impact on the nutritional composition of human milk.
The aim of this study was to explore the correlation between the nutritional status of human milk donors at the Northern Human Milk Bank and the nutritional composition of donor human milk (DHM).
Methods: A cross-sectional study was conducted in which 94 donors were evaluated. Nutritional status was assessed by weight, height, body mass index (BMI), percentage of fat mass (%FM), waist circumference (WC), and hip circumference (HC). The nutritional composition of raw DHM was analyzed using the Human Milk Analyzer (MIRIS, Uppsala, Sweden), a device that provides information on energy (kcal/100mL), crude protein, true protein, carbohydrates and fat (g/100mL).
Results: The median age of donors was 33 years (min=21; max=43), with 40.4% of donors categorized as overweight and 13.8% as obese. The mean %FM was 32.7%. The mean WC was 88.6±11.4cm, and the mean HC was 109.0±11.5cm.
No statistically significant correlations were identified between BMI and the nutritional composition of DHM. Regarding the correlation between %FM and the nutritional composition of DHM, a positive correlation was found with energy (r=0.217, p=0.048), crude protein (rs=0.234, p=0.032), true protein (rs=0.217, p=0.048), and fat (rs=0.241, p=0.027).
A positive correlation was observed between WC and energy (r=0.463, p=0.015) and fat (rs=0.507, p=0.007). Regarding HC, a positive correlation was identified with fat (rs=0.417, p=0.031).
Conclusion: The results suggest the presence of a relationship between the human milk donors body composition and the nutritional composition of human milk, particularly regarding energy and fat. Despite the need for an increased sample size, the preliminary results highlight the importance of nutritional monitoring of mothers to optimize the nutritional composition of breast milk.
