Rationale: Malnutrition is a significant concern among patients undergoing chemotherapy, with a reported prevalence of 24–74% in Sri Lanka. Recommended nutritional intake is an energy of 25–30 kcal/kg/day and protein >1 g/kg/day–1.5 g/kg/day. The objective of this audit is to assess the adequacy of calorie and protein intake among patients undergoing chemotherapy at the Medical Nutrition Unit, Apeksha Hospital Maharagama, and to assess their nutritional status.

Methods: A total of 40 patients receiving chemotherapy between 01.06.2025 and 30.06.2025 were assessed on calorie and protein intake according to Espen guideline on nutrition in cancer patients 2021. A 24-hour dietary recall was used to estimate calorie and protein intake using Nutri Survey software. Nutritional status was evaluated using BMI and percentage weight loss.

Results: Only 8 patients (20%) achieved the recommended calorie intake. Protein intake was markedly inadequate, with only 1 patient (2%) meeting the recommended intake of > 1g/kg/day-1.5 g/kg/day. The majority, 31 patients (77%), consumed less than 1.0 g/kg/day and 8 (20%) had an intake of 1g/kg/day. Loss of appetite was the most common contributing factor (50%). Although 21 patients (52%) had a normal BMI, malnutrition was prevalent, with 20 (50%) severely malnourished and 8 (20%) moderately malnourished.

Conclusion: Most patients failed to meet recommended calorie and protein requirements, with a high prevalence of malnutrition despite normal BMI. Early nutritional assessment and targeted interventions are essential to improve clinical outcomes.
