A COST-EFFECTIVENESS ANALYSIS OF 100% WHEY PEPTIDE-BASED ENTERAL NUTRITION IN US ADULTS FOLLOWING HOSPITAL DISCHARGE

Rationale: In patients depending on home enteral nutrition (HEN), peptide-based formulas are essential for those with malabsorptive conditions, gastrointestinal dysfunction or with intolerance to standard polymeric formulas (PF). The objective of this analysis is to assess cost-effectiveness of 100% whey peptide-based (100%-WP) formulas (Peptamen®) vs. PF in US adult HEN patients after hospital discharge.

Methods: A Markov model with daily cycles over 12 months was developed from the US healthcare system perspective. Costs and life days were simulated for each health state – stable health, oral autonomy, healthcare visits and inpatient stays. Model inputs were derived from US-specific data sources.

Results: Outcomes were reported per patient-year. Patients receiving 100%-WP experienced 19 additional days in a stable, complication-free health state compared to those receiving PF (221 vs. 202 days). A greater proportion of patients in the 100%-WP group also transitioned from home enteral nutrition to oral autonomy. Use of 100%-WP was associated with reduced healthcare utilization, including fewer healthcare visits (42 vs. 67) and fewer inpatient days (14 vs. 21) compared to PF. These reductions corresponded to significant (p < 0.001) cost savings across all indexed measures, estimated at approximately $22,500 from fewer healthcare visits and $88,531 from reduced inpatient stays. Overall, 100%-WP was associated with greater complication-free time and total cost savings of $90,258 compared to PF.

Conclusion: For US adult HEN patients for whom 100%-WP is indicated, 100%-WP demonstrates superior effectiveness post-discharge, offering improved clinical outcomes and significant annual cost savings compared to PF, primarily driven by reductions in complications and inpatient stays.
