CENTRAL OBESITY - CAN A MEDICAL INTERVIEW COMPLEMENT THE LIMITATIONS OF PHYSICAL MEASUREMENT?

Rationale:  When taking physical measurements during clinical practice,we occasionally acknowledge the discrepanciy between the value of CT-measured visceral fat area(VFA )and waist circumference(WC),body mass index(BMI).This makes it difficult to diagnose central obesity at an early stage,so we examined the usufulness of a medical interview-based approach for the issue.

Methods: 5 years' worth of annual health check up results were used(2020-2024,total of 58,772 people  male 34,116 female 24,656, 0-94y.o.). We checked those who choose visceral fat CT test,190 cases(male 147 female 43, age 18-74)and divided them into 2 groups- groupA(VFA>=100cm2 ) and groupB(VFA<100cm).From pre-examination questionnaire,we used 4 items.Item C- eat faster than others,ItemD- weighing 10kg more than at the age of 20y.o., Item E take supper within 2hours before sleeping, and Item F exercise(2 or more times a week 2points,less than 2times a week 1point, none 0). GroupA,B versus ItemC,D,E-Fisher test was performed.GroupA,B versus ItemF- Cochran‐Armitage test was performed.(P<0.05)

 

Results: Of the total190cases, participants undergoing treatment for hypertention,diabetes and  dyslipidemia were excluded.Finally male 92cases were

examined(32-74y.o. VFA  22.9-211.8cm2 ).Female were only 24cases,so we have discontinued the analysis.P-value for each test were as follows;

GroupA,B vs. ItemC P=0.001918.GroupA,B vs.ItemD P=0.00000116.GroupA,B vs.ItemE P=0.77.GroupA,B vs. ItemF P=0.0279.In addition 11cases in GroupA showed a discrepancy between VFA and 'BMI and/or WC', similarly 8 cases in GroupB.In groupA,ItemC and D provided a clue to solve the issue in 8cases,and in GroupB  Item C,D and F provided a clue to solve the issue in all cases.

Conclusion: For early detection of central obesity, some medical interviews may be useful in various clinical settings.
