NUTRITIONAL STATUS IN INTERSTITIAL LUNG DISEASES TRANSPLANT CANDIDATES

Rationale: Malnutrition increases morbimortality in patients with interstitial lung diseases (ILD) who are candidates for lung transplant (LT). This study aims to determine the prevalence of malnutrition (GLIM criteria) and sarcopenia (EWGSOP2 criteria) in LT candidates with ILD and to describe their clinical characteristics.

Methods: Prospective, longitudinal study, conducted between 2021 and 2024 in patients with ILD listed for LT. Nutritional assessment included anthropometric measurements, bioelectrical impedance analysis and functional tests (dynamometry, Timed-Up and Go and 30 second chair stand tests).

Results: Of the 95 patients evaluated, 46.4% were diagnosed with malnutrition based on GLIM criteria. The most frequent phenotypic criterion was unintentional weight loss, which was significantly more prevalent in the malnourished group (88.6% vs. 30.6%; p<0.001). Only 4.2% had a low BMI, while overweight (41.1%) and obesity (31.6%) were more prevalent. Sarcopenia prevalence in this cohort was 10,53%, with 50% of the identified cases classified as sarcopenic obesity.

Malnourished patients exhibited lower body weight (71.14 kg vs. 79.54 kg; p=0.004), BMI (25.42 kg/m² vs. 28.72 kg/m²; p<0.001) and reduced arm (29,54 cm vs. 31,74 cm; p=0,009) and calf circumferences (35,47 cm vs. 37,32 cm; p=0,009). Additionally, they showed an unfavorable body composition: lower fat mass (21.51 kg vs. 27.79 kg; p<0.001), fat free mass index (17,63 kg/m² vs. 18,57 kg/m²; p=0,027) and appendicular skeletal muscle mass index (6,61 kg/m² vs. 7,01 kg/m²; p=0,046). No significant differences were found regarding age or sex, hand grip strength, functionality or biochemical markers.

Conclusion: Malnutrition, sarcopenia and sarcopenic obesity are prevalent conditions in LT candidates with ILD. Their presence is associated with adverse changes in body composition that may not be evident through BMI alone.
