METABOLIC DISORDERS AND GUT MICROBIOTA: INSIGHTS OF A DIETARY INFLAMMATORY INDEX (RDII) IN FASTING INDIVIDUALS

Rationale: Low-grade inflammation is contributor of metabolic disorders, the role of diet-microbiota interaction is investigated.In this study,we used a dietary inflammatory index(rDII) to explore this interaction with inflammation and insulin-resistance in fasting individuals.
Methods: This prospective study was conducted among volunteers during a fasting-period.A simplified dietary inflammatory index(rDII) was calculated using weekly-frequency of sixteen-nutriments(8 proinflammatory and 8 anti-inflammatory),with pro-inflammatory componets assigned positive values and anti-inflammatory componets negative values.Fasting-glucose and insulin were used to compute the Homeostasis Model Assessment of insulin-resistance(HOMA-IR). High sensitivity C-reactive-protein(hs-CRP) was used as inflammatory biomarker.
Associations were assessed using Spearman correlation.
Results: A total of 1174 participants were included. The mean of age was 50,25±13,77years and female-to-male-ratio was 2.
During the fasting period, the median of HOMA-IR was 3.16(IQR:1.95-5.19),and the median of hs-CRP was1.85mg/L(IQR:0.92-3.66).
Compared with pre and post-fasting periods,rDII was higher during fasting period,indicating a more pro-inflammatory dietary-pattern with median of -1.5 (IQR:-2.5to-0.5)(p<10⁻³),while the median HOMA-IR suggested a tendency toward insulin-resistance.
Furthermore,a significant positive correlation was observed between HOMA-IR and rDII(r=0,144;p=0,007) as well as between HOMA-IR and hs-CRP(r=0,249; p=0,001).
Conclusion: Our findings suggest that,despite expected benefical fasting benefits,a more pro-inflammatory dietary pattern may contribute to insulin-resistance as marker of metabolic dysfonction.This association may be mediated by interaction with gut-microbiota,whereby its dysregulation may induce low-grade inflammation.This highlightes the link between lifestyle factors,microbiota,and metabolic disorders.
