Rationale: In Western populations, the Mediterranean diet, which has a lower dietary acid load (DAL), is associated with a lower risk of chronic kidney disease onset and progression. However, evidence regarding Asian diets is limited. In this study, we examined whether the Japanese dietary pattern is associated with DAL, dietary inflammatory potential, and the urinary sodium-to-potassium (Na/K) ratio.
Methods: This cross-sectional study utilized data from a Japanese cohort of 6,274 community-dwelling adults aged ≥40 years (median age, 69 years; 49.1% men) who provided spot urine samples and responded to a food frequency questionnaire. Adherence to the Japanese dietary pattern was assessed using the 8-item Japanese Diet Index (JDI8). DAL was estimated from potential renal acid load (PRAL) and net endogenous acid production (NEAP), while dietary inflammatory potential was assessed using the Dietary Inflammatory Index (DII). Urinary Na/K ratio was calculated from spot urine samples. Correlations between the JDI8 and these three indices were assessed, and multivariable linear regression models simultaneously including all eight JDI8 food items were used to evaluate the associations of each item with PRAL, NEAP, and DII.
Results: JDI8 score showed modest inverse correlations with PRAL (ρ = −0.345), NEAP (ρ = −0.319), and DII (ρ = −0.502), while that with spot urinary Na/K ratio was weakly positive (ρ = 0.028). In the multivariable analyses of individual JDI8 components, green and yellow vegetables showed the largest inverse associations with PRAL, NEAP, and DII in each model, based on standardized beta coefficients, relative to positively associated components such as beef and pork.
Conclusion: Higher adherence to the Japanese dietary pattern is correlated with a lower DAL and DII, and green and yellow vegetables are potential main contributors to that correlation.
