SUBJECTIVE GLOBAL ASSESSMENT AND MORPHOFUNCTIONAL CORRELATES IN PATIENTS WITH AMYOTROPHIC LATERAL SCLEROSIS (NUTRIELA PROJECT)

Rationale: Malnutrition is highly prevalent in amyotrophic lateral sclerosis (ALS) and is associated with poorer functional outcomes. This study aimed to evaluate whether Subjective Global Assessment (SGA) categories are associated with morphofunctional parameters and clinical functionality in patients with ALS.

Methods: Ambispective multicenter observational study including patients with ALS. Nutritional status was classified according to the Subjective Global Assessment (SGA) as well-nourished, moderately malnourished, or severely malnourished. Bioelectrical impedance vector analysis (BIVA) and ultrasound parameters were compared across groups using one-way ANOVA with post-hoc analysis. Associations between variables were assessed using Pearson’s correlation.

Results: A total of 334 patients were included (mean age 64.3±11.6 years; 55.1% male), with a mean ALSFRS-R score of 33.4±9.85. According to SGA, 51.4% were well-nourished, 26.2% moderately malnourished, and 22.4% severely malnourished.

A progressive decline from well-nourished to severely malnourished patients was observed in phase angle (4.47 vs 4.33 vs 3.90; p<0.001), body cell mass (22.17 vs 19.88 vs 17.38 kg; p<0.001), handgrip strength (19.9 vs 16.0 vs 14.4 kg; p=0.022), rectus femoris cross-sectional area (4.09 vs 3.54 vs 3.05 cm²; p<0.001), and rectus femoris thickness (1.30 vs 1.13 vs 0.95 cm; p<0.001). ALSFRS-R scores were correlated with all these parameters (Figure 1).

Conclusion: Worse nutritional status, as defined by SGA, is associated with progressive impairment in phase angle, body cell mass, muscle strength, and ultrasound-derived muscle parameters in patients with ALS. Their significant correlation with ALSFRS-R supports the potential role of these morphofunctional measures as complementary markers for integrated nutritional and functional assessment in this population.
