Rationale: Prediabetes affects 5.8% of adults globally (298 million), with overweight being a key risk factor. The American Diabetes Association recommends a 7–10% weight reduction to mitigate progression to type 2 diabetes. This study evaluates outcomes of a Hong Kong government-led, dietitian-supported chronic disease management program targeting overweight prediabetic individuals.

Methods: Among 120 patients referred for dietary intervention (January–October 2024), 23 completed two follow-ups within six months; 17 met inclusion criteria (HbA1c >5.7% or fasting glucose >5.6 mmol/L, BMI >24 kg/m²). Body composition (weight, body fat %, muscle mass) was assessed via bioelectrical impedance analysis (BIA) at baseline and post-intervention. Paired t-tests compared pre- and post-intervention changes.

Results: Participants (mean age 57.6 ± 7.5 years, baseline BMI 27.0 ± 2.6 kg/m²) exhibited significant reductions in weight (70.8 ± 13 kg to 68.6 ± 11.5 kg, p=0.007) and body fat (34.7 ± 6.4% to 26.2 ± 2.1%, p<0.001). Muscle mass remained unchanged (43.9 ± 10.3 kg vs. 43.7 ± 10.3 kg, p=0.281).

Conclusion: Dietitian interventions in this program significantly improved weight and body composition in overweight prediabetic patients without compromising muscle mass, supporting its role in metabolic health management in community setting.
