IMPACT OF A HIGH-PROTEIN NUTRITION SHAKE ON DIETARY INTAKE IN GLP-1 RECEPTOR AGONISTS (GLP-1RA) ADULTS

Rationale: GLP-1RAs cause weight loss but are associated with lean body mass loss and malnutrition.1 We evaluated the impact of an organic, plant-based High Protein Nutrition Shake (HPNS) on macronutrient intake in individuals being treated with GLP-1RAs. 

Methods: A single-arm, open-label pilot study evaluated 20 adult participants (16 f, 4 m) taking GLP-1 RA injections. Following a 3-day baseline, subjects underwent a 7-day intervention where they were instructed to consume 1 to 2 cartons of the HPNS daily. Dietary data were collected via 24-hour recalls at baseline and on Day 8. Dietary data were analyzed by ESHA's Food Processor®, and statistical comparisons were conducted via two-tailed paired t-tests. 

Results: Protein and fiber intake was significantly increased (66 ± 4 g v 110 ± 8 g and 12 ± 1 g vs 22 ± 1 g, both p < 0.001), while total energy (1419 ± 122 kcal vs 1530 ± 135 kcal), carbohydrate (156 ± 15 g vs 135 ± 17 g), and fat (60 g vs 65 g and 37.2 ± 2% vs. 37.2 ± 2%) were unchanged.   Carbohydrate intake decreased from 44% ± 2% to 35% ± 1% of total daily calories (p = 0.005), concomitant with the increase in dietary fiber and significant reduction in sugar intake (54.9 g vs.35.5 g, p=0.005).

Conclusion: Consumption of this HPNS improves dietary quality in individuals undergoing GLP-1RA treatment by significantly increasing total protein and dietary fiber intakes. Daily consumption of the shake shifts the dietary profile toward a higher protein-to-carbohydrate ratio without causing excessive total caloric consumption. These findings represent a nutrition-focused strategy to mitigate protein and fiber deficits while optimizing metabolic and clinical outcomes in this population.
