RELEVANCE OF OBESITY AND SARCOPENIC OBESITY IN PATIENTS WITH CHRONIC KIDNEY DISEASE

Rationale: Obesity is very prevalent in patients with CKD and the sarcopenic form of obesity can go unnoticed and increases morbidity and mortality in patients with CKD.

Objective. To assess the prevalence of obesity and sarcopenic obesity (SO) in patients with CKD, analyzing the differences between body composition parameters between the group with SO and those without SO.

Methods: A total of 348 patients with CKD (advanced CKD and hemodialysis) were evaluated, we have considered demographic parameters such as age and gender, as well as weight, height, BMI, and body composition with segmental multifrequency BIA Inbody S10. Obesity % fat mass is defined >25 in men and 35% in women, low muscle mass if the SMI skeletal muscle mass index < 7 in men and 6 in women. Body composition parameters such as cell mass, fat mass are analyzed. Fat-free mass, phase angle, AEC/ACT ratio, visceral fat area, waist circumference, arm muscle circumference.

Results: Of the 348 patients, 71.69 ± 12.28 years whith x%Fat mass 34.22 ± 9.56, and VFA 131.21 ±55.87.
258 (74.1%) patients were found to be obese, 191 (82%) of whom were men and 67 (58.3%) of women. A total of 67 patients (19.3%) had low SMI, 35 (15%) of whom were men and 32 (27.8%) of women.
Of the obese patients, 50 (19.4%) had sarcopenic obesity, 19 women and 31 men. The patients with SO 44,4% on Advanced CKD and 55,6% in HD (p0.183)
The results of the different significant parameters are shown in the table below.

Conclusion: 1.- Obesity is very prevalent in patients with CKD, without differences between Advanced CKD or HD in our study
2.- Sarcopenic obesity appears in a low but not negligible percentage and alters body composition parameters that influence mortality
3.- Making an early diagnosis helps to establish the appropriate strategies to correct obesity as a cardiovascular risk factor and sarcopenic obesity that can go unnoticed if weight is maintained.
