EFFECTIVENESS OF OXANDROLONE IN PROMOTING MUSCLE GROWTH IN PATIENTS WITH SEVERE BURN

Rationale: Oxandrolone is an anabolic-androgenic hormone that promote muscle growth and weight gain following burns. A systematically done review of literature will benefit collating the synthesized effects of oxandrolone on related clinical parameters.
Methods: Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines were followed. PubMed, Google Scholar, HINARI databases were searched. Screening of articles was done with four questions; presence of primary data, having quantifiable outcomes, outcomes being related to muscle growth and whether the study included burn patients. In the first-round abstracts and titles were searched and in the second-round, full articles were searched. Risk of bias assessments were done. Due to clinical and methodological heterogeneity, pooled analyses were not done
Results: Six articles were selected, five being interventional and one being a prospective cohort. There were 568 participants with 523 children (5 studies) and 45 adults (1 study). The outcomes included serum assays (2 studies), anthropometry (2 studies), muscle protein/strength assessments (3 studies), scar assessments (1 study), cardio-pulmonary capacity assessments (1 study) and patient reported outcomes (1 study). Favorable outcomes were observed for; protein balance (p<0.05), lean body mass improvements (p<0.05), scar pliability (p<0.05), scar severity (p<0.05), muscle strength (p<0.05) and emotional health (p<0.05) in the oxandrolone-group. One study reported a no-difference of proponolol concentration with concomitant administration of it. When combined with exercises (1 study), superior outcomes were observed.
Conclusion: Administration of oxandrolone shows favorable outcomes among survivors of burns in relation to protein level, muscle level, anthropometry-level, scar-level and patient-reported-outcome level outcomes. Potential Synergistics effects with exercises was also observed.
