Rationale: Adherence to oral nutritional supplements (ONS) is often suboptimal early in hospitalization, limiting nutritional therapy effectiveness. This study evaluated if repeated immersive virtual reality (VR) education could improve ONS adherence.

Methods: This prospective controlled study included hospitalized adults prescribed ONS. The intervention group received daily immersive VR education via head-mounted display starting within 24 hours of prescription; controls received standard care. The primary outcome was daily ONS adherence (% prescribed volume consumed). Propensity score matching (PSM) balanced baseline characteristics (age, sex, BMI, NRS-2002). Longitudinal analysis used Generalized Estimating Equations (GEE).

Results: After PSM (n=52), adequate adherence (≥75% prescribed volume) was achieved by 88.5% (23/26) in the VR group vs. 61.5% (16/26) in controls (p=0.02). Superior adherence in the VR group was significant during the first three hospitalization days (GEE, p=0.017).

 

Conclusion: Repeated immersive VR education was associated with significantly higher early ONS adherence in hospitalized adults. This feasible tool can be integrated as an adjunct to standard nutritional care to improve patient engagement and early nutritional intake.
