Rationale: Cohesion to the European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines is important to optimize clinical outcomes and minimize complications related to parenteral nutrition (PN), and ensuring the safety and efficacy. However, clinical practice frequently varies from the validated guidelines. This study aimed to develop a questionnaire based on ESPEN guidelines to audit PN use in hospitalized patients.
Methods: A questionnaire was developed by extracting key clinical recommendations from the most recent ESPEN guidelines that concerns the administration and use of PN. Recommendations were translated into objective closed-ended questions to ensure objective measurement and ease of data analysis. A closed-loop response system was used to limit answers and guarantee consistency. Patients were categorized by medical condition and profile, ensuring a tailored evaluation of PN indications, formulations, and monitoring.
Results: The developed questionnaire contains a wide range of clinical domains including general PN management (assessment, micronutrient supplementation, and metabolic monitoring) and specialized sections for critically ill, oncology, geriatric, and surgical patients. It also incorporates disease-specific criteria for conditions, namely chronic intestinal failure, pancreatitis, and renal or liver failure. This structured format facilitates the identification of specific areas where documentation or clinical practice fails, or derivates from guidelines.
Conclusion: This questionnaire provides a scalable framework for quality improvement in different patient populations receiving PN. It enables healthcare institutions to identify specific gaps between clinical practice and the ESPEN provided guidelines.
