MALNUTRITION ON INTENSIVE CARE UNIT-ACQUIRED WEAKNESS PATIENTS FROM A PUBLIC HOSPITAL IN ARGENTINA

Rationale: Intensive Care Unit-acquired weakness (ICUAW) can be due to multiple reasons, among them, there are the ones related to the nutrition state of the patient. The objective of this study was to describe the nutritional status of patients with ICUAW.

Methods: The single-center observational analytical study of the retrospective cohort of a public Argentine hospital included patients aged 18 and older. We analyzed the nutrition diagnosis upon admission using Subjective Global Assessment (SGA) and Body Mass Index (BMI), early initiation of feeding (≤ 48 h), and requirement for parenteral nutrition (PN) during the first week in Intensive Care Unit (ICU) in patients with ICUAW (handgrip strength - Jamar reference: <11 kg in men, <7 kg in women at ICU discharge). p < 0.05 considered statistically significant. 

Results: 45 patients (aged 14 to 54) were evaluated. 53.3% were male. The average stay was 11.2 days, 31.1% of the patients required mechanical ventilation. 24.4% (n = 11) displayed ICU-AW (mean 9.25 kg for men and 4.14 kg for women), 90.1% of these patients presented severe malnutrition (p<0.001) compared to patients without ICUAW. Regarding BMI, 45,4% were within a normal range, 18.2% were overweight, and 36.4% suffered from obesity (p 0.809). 45.6% initiated early nutrition (p 0.257). Notably, half of ICUAW patients required PN during the first week (45.6%; p 0.015).

Conclusion: Severe malnutrition upon admission would be one of the key factors in ICUAW, whereas BMI is not. Early nutrition was not a protective factor in the analysed sample, and the use of PN during the first week significantly increased the risk.
