SARCOPENIA IN CIRRHOSIS: THENAR VS RECTUS FEMORIS SKELETAL MUSCLE ULTRASOUND (SMUS) COMPARISON.

Rationale: Assess sarcopenia prevalence in (de)compensated cirrhosis using SMI and analyse the association with SMUS. We compared lower and upper limb to evaluate SMUS thenar as a non-invasive, low cost, time-efficient tool to follow-up muscle mass.

Methods: In 84 cirrhosis patients, SMUS according to the SARCUS protocol1, was compared to CT-derived skeletal muscle mass index (SMI) at L32, or to MRI if no CT available (8 patients)3. We measured muscle thickness (RF MT) and cross-sectional area of rectus femoris (RF CSA) and MT of thenar transverse (TT), whole transverse (WTT) and longitudinal (LT).4,5 

Results: Sarcopenia was prevalent in 49.4% of the cohort (63.1% men, median age 61 years [IQR 48.5-67], mean Child-Pugh score 7 [IQR 5-9]). In males, SMUS of both upper and lower limb showed a statistically significant association with CT SMI. MT RF in men had an AUC of 0.73 for diagnosing sarcopenia. WTT and LT correlated with MT RF in both sexes. (Table 1)

Conclusion: Sarcopenia is highly prevalent in cirrhosis; MT RF can be used as a screening tool. SMUS of both upper and lower limb seem reliable alternatives to CT/MRI especially in males. In women LT and WTT can be radiation-free convenient tools to assess sarcopenia. These findings may provide practical and accessible approaches for diagnosing sarcopenia in cirrhosis.
