Rationale: To evaluate the effectiveness of a structured perioperative nutritional prehabilitation program in improving clinical, functional, and nutritional outcomes in patients undergoing oncologic surgery.
Methods: This prospective cohort study at a tertiary oncology hospital included 10 adult patients undergoing major GIcancer surgeries. Nutritional status was assessed using NRS-2002, SGA and a 3-day diet recall, body composition was measured via BIA. Biochemical parameters (CBP, serum albumin) and quality of life (EORTC QLQ-C30) were recorded at baseline, preoperatively and on postoperative days 7 and 30. Patients received individualized dietary counseling and nutritional supplements with postoperative adherence support. Changes over time were analyzed using repeated measures ANOVA.
Results: Skeletal muscle mass increased significantly (p<0.05), indicating preservation and improvement of lean mass with nutritional care. Fat mass index and body fat percentage decreased (p<0.05), reflecting fat loss likely due to postoperative intake reduction and surgical stress. Body weight declined gradually. Food intake reduced postoperatively but improved by 3 months. Quality of life decreased initially, then significantly improved (p<0.01) at 3 months, highlighting the overall benefits of structured nutritional support.

Conclusion: Nutritional prehabilitation and follow-up support enhance weight, body composition and quality of life in surgical oncology patients.
Individualized care and early rehabilitation are essential. Despite fluctuations in clinical parameters, overall quality of life improved consistently.
