ASSOCIATION OF ANTHROPOMETRIC INDICES WITH ATHEROGENIC INDEX OF PLASMA AND NON-HDL CHOLESTEROL IN PATIENTS WITH TYPE 2 DIABETES MELLITUS

Rationale: Atherogenic dyslipidemia (evaluated by the atherogenic index of plasma (AIP) and non-HDL-C) increases cardiovascular risk in type 2 diabetes mellitus (T2DM). This study aimed to investigate the associations of BMI, WC, and WHR with AIP and non-HDL-C in patients with T2DM, and to evaluate their potential utility as simple anthropometric indicators of atherogenic risk among Vietnamese inpatients.
Methods: A cross-sectional study was conducted in 149 patients with T2DM at a hospital in Thanh Hoa, Vietnam. Patients with acute complications of T2DM or critical illness were excluded. Body mass index, waist circumference, waist-to-hip ratio, and serum lipid parameters were collected. AIP was calculated as log(TG/HDL-C) [1]. Multivariable linear regression and ROC analysis were used to examine associations and discriminatory performance.
Results: BMI, WC, and WHR were independently associated with AIP, whereas only WC and WHR were significantly associated with non-HDL-C after adjustment (ß coefficient values for WHR and WC were 4.857 and 0.028, respectively; R-square values were 0.135 and 0.121). The ROC analysis showed modest performance, with BMI best identifying elevated AIP (AUC=0.701) and WHR best identifying high-risk non-HDL-C (AUC=0.630).

Conclusion: Anthropometric indices were significantly associated with atherogenic lipid abnormalities in Vietnamese patients with T2DM. While all three indices were related to AIP, WC, and WHR appeared more informative for non-HDL-C. These findings support the potential role of simple anthropometric measures in low-cost cardiometabolic risk screening, particularly in resource-limited settings.
