PROGNOSTIC IMPACT OF BODY COMPOSITION AND SARCOPENIA PARAMETERS ON SURVIVAL IN CERVICAL CANCER PATIENTS TREATED WITH CURATIVE CHEMORADIOTHERAPY

Rationale: Body composition parameters(BCPs),sarcopenia and adipose tissue distribution,are increasingly recognized as prognostic factors in oncology;however their role in cervical cancer(CC) remains unclear.This study evaluated the association between BCPs and survival in CC patients.
Methods: We analyzed 96 patients treated with CRT(median EBRT dose 50.4Gy[45–59.4]),followed by brachytherapy.Pre-treatment computed tomography images at the L3 level were used to manual contouring for skeletal muscle area(SMA), subcutaneous adipose tissue area(SAT) and intra-abdominal compartment area(IACA).SMI(SMA/height²), SAT/IACA and (SMA+SAT)/IACA were calculated.Kaplan–Meier and Cox regression were used;p<0.05 was significant.
Results: Median age was 52.Median follow-up was 31months(9–106).51% of patients had FIGO IIIC1.The median body mass index(BMI) was 28.2kg/m².The 2- and 5-years CSS were 89.7% and 74.8%,while OS were 88.5% and 70.4%.All BCPs(SMA, SMI, IACA, SAT/IACA and (SMA+SAT)/IACA)) were significant in univariate analysis,except for SMA/IACA(p=0.22).BMI was also not significant.In multivariate analysis, low SMA(<110 cm²) was associated with increased mortality(HR:3.45,p=0.02 for CSS; HR:3.13,p=0.02 for OS).Low SMI(<41 cm2/m2) was significantly associated with worse CSS(HR:4.62,p=0.004) and OS(HR:4.42,p=0.004).Increased IACA (>160 cm²) was associated with worse CSS(HR:3.4,p=0.04) and OS(HR:3.52,p=0.042).Unfavorable fat distribution (SAT/IACA <1) was associated with poorer CSS(HR:3.0,p=0.042) and OS(HR:3.54,p=0.016).Additionally, (SMA+SAT)/IACA<1.59 was associated with poorer CSS(HR:3.51,p=0.03) and OS(HR:4.19,p=0.012).

Conclusion: Both SMA and SMI emerged as independent prognostic factors. Adipose tissue distribution also played a critical role; lower SAT/IACA and (SMA+SAT)/IACA associated with poorer survival. These metrics may support personalized treatment strategies.
