THE EFFECT OF EMOTIONAL STATUS ON DIETARY INTAKE IN ADULTS WITH AXIAL SPONDYLOARTHRITIS

Rationale: Patients with axial spondyloarthritis (axSpA), a chronic inflammatory rheumatic disease mainly presented with spinal discomfort, may frequently experience emotional disturbances such as anxiety and depression. These emotional symptoms may affect dietary intake. However, there is not enough evidence related to this topic in axSpA. Therefore, this cross-sectional study aimed to evaluate the effect of emotional status on dietary intake in adults with axSpA.

Methods: Fifty adults (≥18 years) who were followed in the rheumatology outpatient clinic of a university hospital were included. Emotional status was assessed using the Hospital Anxiety and Depression Scale (HADS). Patients were grouped as with and without anxiety (HADS-A ≥10) and depression (HADS-D≥7). Dietary intake was evaluated using a three-day food record, from which dietary antioxidant capacity was estimated based on Ferric Reducing Antioxidant Power (FRAP) values.

Results: Twelve participants (24%) had anxiety, and 15 participants (30%) had depression. Patients with anxiety had significantly higher body mass index (BMI) and lower dietary intake for all the nutrients investigated (Table, p<0.05). Participants with depression similarly had higher BMI and lower dietary intake, but the differences did not reach a significant level (Table, p>0.05), except niacin (Table, p<0.05).

Conclusion: According to our results, anxiety has a clear negative effect on nutritional status in adults with axSpA. Even though depression has also a detrimental effect on dietary intake, future larger studies are needed to reveal its true effect. Nevertheless, the emotional status needs to be considered for planning the nutritional management of this population.
