PATIENT EDUCATION IN HEART FAILURE

Rationale: Patients hospitalized with heart failure (HF) are monitored by a multidisciplinary team, and nutritional counseling is part of the Patient Education Care. The aim of this study is to demonstrate the rate of nutritional counseling during hospitalization.
Methods: The nutritional counseling rate is analyzed monthly. Because the low results in 2023, actions were taken to improve the counseling rate. Information about patients with an indication for nutritional counseling was provided by email once a week. From the end of 2023, in addition to information via email, patients with an indication for counseling were informed daily, via message and supervised by a dietitian responsible for the team.
Results: in 2023, the average nutritional counseling rate was 21.1% (ranging from 0 to 62.1%). The actions taken led to a significant improvement in the following year. In 2024, the average increased to 72.4%, ranging from 56.8 to 81.1%, reaching the target of 60% counseling. In 2025, at the moment with 75.6%, ranging from 73.3 to 78.8%. The actions were fundamental for the American College of Cardiology Accreditation achieved in November 2024. The nutritional counseling, which are part of patient education (nutritional counseling, nursing counseling, medical counseling -follow-up consultation, days for follow-up consultation, guidance on rehabilitation) and weight monitoring, medications and exams, contributed overall to the reduction of the average mortality rate from 12.5% in 2023 to 8.6% in 2024. Regarding readmission within 30 days due to HF, there was an improvement from 8.7% to 7.6%.
Conclusion: the actions carried out contributed to the adequacy of the nutritional counseling rate for patients with HF. The 2025 target was increased to 70% adequacy.
