Rationale: Older adults receiving home care demonstrate reduced physical function and nutritional status.  The POWER randomized controlled trial investigated the effects of a 12-week whey protein oral nutritional supplement plus online resistance training (ONS + RT), compared to RT alone on physical function, nutritional status and health-related outcomes. 

Methods: Older adults (n=46, ≥70 years) receiving home care, at risk of sarcopenia (SARC-F ≥1), were randomized into (i) ONS + RT: ONS x2/day and online RT twice/week or (ii) RT: online RT twice/week only.  Primary outcomes were Timed Up & Go (TUG) and Mini-Nutritional Assessment-Full Form (MNA-FF).  Secondary outcomes included body composition, protein intake (g/kgBW/day); leucine intake (g/day), gait speed, handgrip strength, Five times Sit-To-Stand, cognitive function, depression risk, activities of daily living and quality of life.  Outcomes were measured at baseline (T1), post- (T2) and 12-weeks post-intervention (T3). 

Results: Both groups significantly improved TUG (Mean[95%CI] ONS + RT: T1=11.3s [9.2-13.8]; T2=10.4s [8.3-13.2]; RT: T1=12.0s [9.9-14.6]; T2=10.8s [8.7-13.5], time effect p=0.028) and MNA-FF (ONS + RT: T1=23.6 [22.2-24.9]; T2= 24.4 [22.7-26.2]; RT: T1=22.4 [21.1-23.7]; T2=24.3 [23.1-25.5], time effect p=0.011) at T2, with no group x time interaction or group effect.  T2 protein intake was higher in ONS + RT than RT (1.3[1.1-1.6] vs. 1.0[0.8-1.1] g/kgBW/day; interaction effect p=0.009) as was leucine intake (4.5[2.9-6.2] vs. 2.2[1.6-2.8] g/day; interaction effect p=0.003).  At T3, RT mean score returned to “malnutrition risk”, while ONS + RT maintained normal nutritional status.

Conclusion: Physical function and nutritional status improved across both ONS + RT and RT groups.
