Rationale: Real-world data on early lifestyle and patient-reported changes during anti-obesity treatment with glucagon-like peptide-1 (GLP-1)-based therapies remain limited. We evaluated short-term changes in anthropometry, weight-related quality of life and lifestyle habits in adults with obesity treated in routine clinical practice.

Methods: Prospective observational study in non-diabetic adults with body mass index (BMI) >27 kg/m² plus cardiovascular risk factors or >30 kg/m² treated with GLP-1-based therapy in an endocrinology unit. Assessments were performed at baseline, 1 month and 3 months. Weight, BMI, waist and hip circumferences, weight-related quality of life, Mediterranean diet adherence and physical activity were analysed using paired parametric tests.

Results: Eighty-two patients were included (age 46.4±11.4 years; 78% women; baseline BMI 39.5±6.0 kg/m²). At 3 months, mean weight decreased by 10.3±4.0 kg (p<0.001), waist circumference by 10.1±6.1 cm (p<0.001), and hip circumference by 8.4±7.2 cm (p<0.001). Weight-related quality of life improved by 12.7±8.8 points (p<0.001). Mediterranean diet adherence increased from 7.9±1.9 to 8.7±2.1 points (p=0.013), weekly moderate-intensity activity days from 0.9±1.7 to 1.8±2.3 (p=0.007), and days with walking ≥10 minutes from 4.4±2.7 to 5.2±2.1 (p=0.011).

Conclusion: : In routine clinical practice, GLP-1-based therapy was associated with clinically meaningful short-term improvements in anthropometric measures and weight-related quality of life in adults with obesity, together with modest favourable changes in Mediterranean diet adherence and physical activity habits.
