ASSOCIATION OF SERUM 25-HYDROXYVITAMIN D WITH HYPERURICEMIA AND METABOLIC SYNDROME IN CHINESE ADULTS

Rationale: Vitamin D plays a role in metabolic disorders, but its relationship with uric acid remains controversial. This study examines associations between serum 25-hydroxyvitamin D [25(OH)D], uric acid, and metabolic syndrome (MetS) in adults.

Methods: Data were collected from 4,228 adults (aged ≥20 years) undergoing physical examinations at a Guangzhou hospital (2015–2020). Measurements included anthropometrics, blood pressure, lipids, glucose, uric acid, creatinine, high-sensitivity C-reactive protein (hs-CRP), and serum 25(OH)D (by HPLC). Hyperuricemia was defined as fasting uric acid >420 μmol/L. MetS was defined per 2009 IDF criteria. Associations were analyzed using logistic regression.

Results: Prevalence of hyperuricemia was 32% (n=1,354) and MetS 36.4% (n=1,538). Vitamin D sufficient (≥75 nmol/L), insufficient (50–75 nmol/L), and deficient (<50 nmol/L) groups comprised 19.3%, 43.6%, and 37.1%, respectively. After adjusting for confounding factors, higher 25(OH)D was positively associated with hyperuricemia risk (P-trend <0.05). Across quintiles, the highest quintile showed a 40.3% increased hyperuricemia risk vs. lowest (P=0.006). This positive association was significant only in women (108.5% increased risk, P=0.005). In contrast, 25(OH)D was inversely associated with MetS risk (P-trend <0.05); compared to the lowest quintile, ORs (95% CI) were 0.86 (0.70–1.05), 0.78 (0.64–0.96), 0.70 (0.57–0.86), and 0.79 (0.64–0.96). Hyperuricemia was associated with a 2.14-fold higher MetS risk (P<0.001). Both conditions correlated with elevated hs-CRP (P<0.01).

Conclusion: Higher serum 25(OH)D levels are associated with reduced MetS risk but increased hyperuricemia risk in Chinese adults, especially women. Further studies are needed to clarify mechanisms and inform vitamin D recommendations. Both hyperuricemia and MetS relate to hs-CRP-mediated inflammation.
