CARDIORESPIRATORY FITNESS AND RISK OF INCIDENT GOUT: A PROSPECTIVE COHORT STUDY FROM THE UK BIOBANK WITH PROTEOMIC ANALYSIS

Rationale: To investigate the association between cardiorespiratory fitness (VO₂max) and incident gout risk and identify potential molecular mechanisms.

Methods: This prospective cohort included 62,714 UK Biobank participants without baseline gout. VO₂max was categorized into quartiles and incident gout identified through hospital records. Cox proportional hazards models estimated hazard ratios adjusting for confounders. Restricted cubic spline analysis examined dose-response relationships. Plasma proteomics (n=3,245) identified differentially expressed proteins associated with VO₂max and gout risk.

Results: Over median 7.8-year follow-up, 473 gout cases occurred. Compared with the lowest VO₂max quartile (≤23.8 ml/kg/min), fully adjusted HRs were 0.89 (0.70–1.13), 0.82 (0.63–1.07), and 0.73 (0.56–0.94) for quartiles 2–4 (P-trend=0.013). Spline analysis confirmed a linear inverse relationship (P-overall=0.003, P-nonlinear=0.599). Protective associations were stronger in men and never-smokers. Higher VO₂max independently associated with lower serum uric acid. Fatty acid binding protein 4 (FABP4) was significantly downregulated in high VO₂max participants and independently associated with increased gout risk (HR=1.48, 95% CI: 1.14–1.92, P=0.004).

Conclusion: Higher cardiorespiratory fitness associates with reduced gout risk and serum uric acid in a dose-response manner. FABP4 may mediate this relationship, suggesting novel prevention and treatment targets.
