ASSOCIATION BETWEEN ENERGY AND PROTEIN ADEQUACY AND DELIRIUM IN SEVERE TRAUMA PATIENTS

Rationale: The relationship between nutritional adequacy and delirium in critically ill patients remains insufficiently explored, despite evidence suggesting a bidirectional interaction (1). Understanding whether energy and protein adequacy affects delirium incidence may impact prevention and outcomes in severe trauma patients.

Methods: In this prospective trauma ICU study, we evaluated the association between energy and protein adequacy and the incidence of delirium in severe trauma patients. Nutritional adequacy was assessed using indirect calorimetry, and patients were classified into adequate and inadequate energy and protein intake groups. Delirium was assessed using CAM-ICU and ICDSC, and groups were compared using appropriate statistical tests.

Results: Among 248 patients, 117 (47.1%) developed delirium. No significant association was found between energy adequacy and the incidence of delirium, as delirium occurred with similar frequency in patients with adequate (70-110%) and inadequate (<70% or >110%) energy intake (p > 0.05). The timing of delirium onset did not differ between groups. Similarly, protein adequacy was not associated with delirium occurrence or time to onset (p > 0.05).

Conclusion: Energy and protein adequacy were not associated with the incidence or timing of delirium in critically ill patients. These findings suggest that overall nutritional adequacy alone may not be sufficient to influence delirium development, supporting further research on targeted nutrition.
