Rationale: Estrogen levels are known to fluctuate during the perimenopausal transition and after menopause, causing wide range of postmenopausal symptoms and health outcomes. However, long-term estradiol trajectory in women with different reproductive lifespan(RL) characteristics has not been fully characterized. This study investigate long-term estradiol trajectories in women with different RL from the UK Biobank, helping clinical physician to optimize estrogen based supplementation stretegy.

Methods: Data were obtained from the UK Biobank, a large population-based cohort that recruited about 500,000 participants aged 40–69 years between 2006 and 2010. In the present study, female participants with available data on menarche, menopause, and estradiol were included. RL was defined as the interval between age at menarche and age at menopause.Mean estradiol levels were evaluated across time before and after menopause, and estradiol trajectories were compared between tertile groups.

Results: Across all tertile groups defined by age at menarche, age at menopause, and reproductive lifespan, estradiol levels rose and reach the top at five years before menopause, peaking at approximately 500–700 pmol/L. Around menopause, Women with earlier menopause and a shorter reproductive lifespan didn't sharply decline and maintain higher estradiol levels after menopause (about 400 pmol/L 10 years after menopause).Whereas, women with earlier menarche tended to have lower estradiol levels before menopause, but showed relatively higher estradiol levels more than 20 years after menopause.

Conclusion: Women with earlier menopause and a shorter reproductive lifespan showed higher estradiol levels across the menopausal transition. These findings suggest that reproductive timing may be associated with distinct long-term estradiol trajectories.
