POST-PERICARDIOTOMY SHOCK: THE INTERSECTION OF CARDIAC SURGERY AND CRITICAL CARE

Rationale: Post-pericardiotomy shock is a complex and potentially fatal form of circulatory failure following cardiac surgery. Its multifactorial nature often complicates diagnosis and delays targeted treatment in the intensive care unit.

Methods: A narrative literature review was conducted using PubMed, including English-language articles published within the last five years. The search focused on pathophysiology, hemodynamic profiles, and diagnostic and therapeutic challenges of post-pericardiotomy shock in critically ill patients.

Results: Post-pericardiotomy shock results from a combination of myocardial dysfunction, systemic inflammatory response, and vasoplegia. Patients frequently require advanced hemodynamic monitoring and multimodal circulatory support. Delayed differentiation of shock mechanisms is associated with worse postoperative outcomes.

Conclusion: Post-pericardiotomy shock represents a critical care challenge at the interface of cardiac surgery and intensive care. Early recognition of its underlying mechanisms is essential for individualized management and improved patient outcomes.
