A CASE-CONTROL STUDY OF THE RATE OF INFECTION IN LUNG CANCER PATIENTS RECEIVING MODERN TOTAL PARENTERAL NUTRITION.

Rationale: Total parenteral nutrition (TPN) is a life-saving supportive therapy when enteral feeding is not feasible in severely malnourished cancer patients. Historically, TPN has been associated with an increased risk of central line–associated bloodstream infection (CLABSI). We aimed to assess whether this association persists in contemporary clinical practice among lung cancer patients.
Methods: We conducted a retrospective 1:1 matched case-control study of adult lung cancer inpatients. Cases included patients receiving TPN within 72 hours of admission, while controls did not receive TPN. Matching was based on demographic and clinical characteristics. Patients with pre-existing bloodstream infection were excluded. CLABSI was defined by positive blood cultures. Nutritional status was assessed using serum albumin.
Results: A total of 42 patients were included (21 TPN, 21 controls). Infection rates were identical in both groups (9.5%, 2/21 patients each; p=1.000). Logistic regression analysis demonstrated no increased risk of infection associated with TPN (OR 1.00, 95% CI 0.11–9.05). Baseline albumin levels were significantly lower in the TPN group compared to controls (2.53 vs. 3.36 g/dL, p<0.001), indicating poorer nutritional status.
Conclusion: Modern TPN use in lung cancer inpatients was not associated with an increased risk of bloodstream infection, despite worse baseline nutritional status. These findings suggest that advances in clinical practice may have mitigated the historically elevated risk of CLABSI. Larger prospective studies are warranted to confirm these results.
