CYSTATIN C-BASED SARCOPENIA INDICES IN TURKISH OLDER ADULTS: CORRELATION WITH BIA PARAMETERS AND COMPARISON WITH SARC-F AND CALF CIRCUMFERENCE

Rationale: Current sarcopenia screening tools show limited sensitivity in clinical practice. The applicability of cystatin C-based indices, creatinine/cystatin C ratio (CCR/SI), predicted skeletal muscle mass index (pSMI), and total body muscle mass (TBMM), has not been evaluated in Turkish older adults. This study investigated their potential as sarcopenia screening tools.
Methods: This ongoing cross-sectional study included 130 community-dwelling older adults attending the Geriatric Outpatient Clinic of Ankara University. SMI and phase angle (PhA) were assessed by multi-frequency segmental BIA. Handgrip strength (HGS), calf circumference (CC), and SARC-F were recorded. CCR/SI, pSMI, and TBMM were derived from fasting serum creatinine, cystatin C, and haemoglobin (Table 1). Sarcopenia was defined per EWGSOP2 2019 with Turkish-specific cut-offs. Spearman correlations were performed.
Results: Mean age was 71.9±6.8 years (women: 65.4%). Sarcopenia prevalence was 7.7% (n=10/130). Mean pSMI was 7.52±0.99 and TBMM 39.33±7.82. SARC-F≥4 was present in 18.0% and low calf circumference in 9.0% of older adults.
Analyses revealed that pSMI and TBMM demonstrated the strongest correlations with BIA-derived SMI (r=+0.658 and r=+0.636, respectively) and HGS (r=+0.515 and r=+0.612, respectively), outperforming both SARC-F and calf circumference (Table 1).
Sex-stratified analyses showed stronger correlations for pSMI and TBMM in men than women (pSMI–SMI: r=+0.620 vs r=+0.383; TBMM–SMI: r=+0.511 vs r=+0.250).

Conclusion: Cystatin C-based indices demonstrated significant correlations with BIA-derived SMI and phase angle, supporting their feasibility as sarcopenia screening tools in Turkish older adults. Population-specific cut-off derivation requires validation in a larger sample.
