TARGETED NUTRITION THERAPY IMPROVES ATHEROGENIC AND CARDIAC BIOMARKERS IN CRITICALLY ILL CARDIAC PATIENTS: A PROSPECTIVE CONTROLLED STUDY

Rationale: Whether targeted medical nutrition therapy (MNT) can improve lipid-related atherogenic markers and cardiac stress biomarkers in critically ill cardiac patients remains underexplored.

Methods: In a prospective controlled study (n=84), ICU patients with acute coronary syndrome or decompensated heart failure were allocated to standard nutrition (SN) or targeted MNT for 14 days. MNT emphasized high-protein (1.5 g/kg), omega-3 fatty acids, soluble fiber (10–15 g/day), phytosterols, and controlled saturated fat (<7% energy). Primary outcomes: LDL-C, Apolipoprotein B (ApoB). Secondary outcomes: Coronary Calcium Score (CCS, baseline only), NT-proBNP, hs-CRP, and phase angle. Statistical analysis included paired t-tests and ANCOVA adjusting for baseline values.

Results: MNT significantly reduced LDL-C and ApoB compared to SN (p<0.01). NT-proBNP and hs-CRP declined significantly in MNT, indicating improved cardiac stress and inflammation. Phase angle improved, suggesting better cellular integrity. No change in CCS was expected over short duration.

Conclusion: Targeted MNT significantly improves atherogenic lipid profile and cardiac stress biomarkers in critically ill cardiac patients. Early integration of structured nutrition protocols may enhance cardiometabolic recovery in ICU settings.
