IMPACT OF A NOVEL INTRAVENOUS LIPID EMULSION ON LIVER DISEASE IN NEONATAL PIGLETS

Rationale: In a previous study1, we showed that a novel (NOV) intravenous lipid emulsion (ILE) can improve the fatty acid profile in blood and tissues of neonatal piglets, meeting the requirement of rapidly growing infants. Here, we report the effects on the liver compared to soybean oil (SO) and mixed oil (SMOF) ILEs to assess if this NOV-ILE could also decrease the risk of intestinal associated liver disease (IFALD). 

Methods: We compared bile flow, serum liver chemistry, and liver histology after 14 days of TPN in neonatal piglets using NOV-ILE (n=8), SO-ILE (n=7), or SMOF-ILE (n=7). Data are presented as median [interquartile range] and comparisons between treatment groups used Kruskal-Wallis (p<0.05).

Results: There were no significant differences in baseline serum liver chemistry (data not shown). Bile flow (µg/kg/min) was significantly increased for NOV-ILE (8.4 [4.3]) versus SO-ILE (3.0 [4.8]), while SMOF-ILE (6.3 [5.8]) was not significantly different to either (p=0.037, Figure 1). Conjugated bilirubin (µmol/L) was significantly decreased for NOV-ILE (1.4 [2.0]) versus SO-ILE (7.4 [6.6]), with SMOF-ILE (3.1 [5.0]) not significantly different to either (p=0.017, Figure 1). Similarly, total bilirubin (µmol/L) was significantly decreased for NOV-ILE (5.0 [7.1]), versus SO-ILE (19.0 [25.0]) with SMOF-ILE not different to either (10.0 [9.6]) (p=0.036, Figure 1). There was no difference in liver histology scores (max 18), NOV-ILE: 1 (1), SO-ILE: 2 (1), SMOF-ILE: 3 (3) (p=0.53).

Conclusion: In this study we demonstrated the novel ILE improved liver function and decreased cholestasis during TPN, suggesting it may prevent IFALD.
