RESTING METABOLIC RATE IN ANKYLOSING SPONDYLITIS: LINKS TO SPINAL MOBILITY, SPINAL POSTURE, DISEASE ACTIVITY, FUNCTIONOL STATUS, AND PHYSICAL ACTIVITY

Rationale: Ankylosing spondylitis (AS) is a systemic rheumatic disease that affects the axial skeleton and is characterized by chronic inflammation, pain, postural changes, and functional limitations. Disease-related inflammation can alter energy metabolism and resting metabolic rate (RMR), by reducing physical activity and causing muscle loss (1). This study aims to determine the relationships among RMR, spinal mobility and posture, disease activity, functional status, and physical activity level in patients with AS.
Methods: Sixty-three patients with AS were included in this study. Spinal posture and mobility, disease activity, functional status, physical activity level, and RMR were evaluated with the lumbar lordosis and thoracic kyphosis angles, Bath Ankylosing Spondylitis Metrology Index (BASMI), Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Bath Ankylosing Spondylitis Functional Index (BASFI), International Physical Activity Questionnaire (IPAQ) and, indirect calorimetry (FitmateR), respectively.
Results: According to the correlation analysis between RMR and clinical and functional parameters, RMR was significantly and positively correlated with BMI (r=0.281, p=0.026) and muscle mass (r=0.634, p<0.001). In contrast, no significant relationships were found between RMR and spinal posture, BASMI, BASDAI, BASFI, and IPAQ (p>0.05).

Conclusion: This study has demonstrated that RMR in patients with AS is specifically associated with muscle mass, but does not show a significant association with other clinical characteristics. Given the potential effects of inflammation on energy metabolism, future studies should investigate the role of the dietary inflammatory index on RMR.
