CORRELATION BETWEEN NUTRITION ACHIEVEMENT RATE AND PROGNOSIS IN MEDICAL ICU PATIENTS

Rationale: Nutritional support is a cornerstone of management for the critically ill. European guidelines recommend reaching 50–70% of caloric requirements within four days of ICU admission. This study evaluates outcomes for patients achieving 50% of caloric and protein needs within the first three days.

 

Methods: This retrospective study analyzed 367 patients in a medical ICU between December 2020 and December 2021. Patients were grouped by whether they met 50% of their nutritional targets defined as 20–25 kcal/kg for calories and 1–1.2 g/kg for proteinwithin three days. Data included demographics, biochemical values (albumin, lactic acid), and clinical outcomes like mortality and length of stay.

 

Results: Of the participants, 229 (62.4%) met the 50% target, while 138 (37.6%) did not. Significant differences (p < 0.05) were found across several domains:Clinical Outcomes: The target-achieving group showed lower mortality and shorter ventilator use.Hospitalization: This group had significantly shorter ICU and total hospital stays.Biochemical/Physical Indicators: Improvements were noted in albumin, prealbumin, hemoglobin, creatinine, and lactic acid levels, as well as grip strength.Severity: Initial disease severity also differed significantly between the groups.

Conclusion: Early achievement of at least 50% of estimated nutritional needs within three days is associated with improved biochemical markers, reduced hospitalization, and lower mortality. These findings highlight early nutritional support as a vital component of intensive care.
